Chronic Disease Management

The Chronic Disease Management program is where clinicians are able to support and monitor patients living with chronic diseases. Our comprehensive approach is designed to empower patients and help them navigate living with a chronic disease. We will help from the moment the patient is diagnosed with the illness to ensure comfort. Each patient will get the ability to have remote monitoring to ensure a clinician can see vitals at any time.

Patients with chronic diseases are able to fill out surveys about how they are doing each day to keep clinicians informed. In addition, patients will record their vital signs giving clinicians the ability to see how they are doing in real-time. Other applications patients can use to help with chronic disease management are the VITAL Health Book with patients videos and VITAL Telehealth to have a meeting with their provider at any time without leaving their homes. We offer personalized care plans for patients and are also able to educate and support them by regularly monitoring and following up.

The Chronic Disease Management program is beneficial and can be life saving for patients. The remote monitoring will ensure more frequent contact for patients and providers without the patient leaving their home. Patients feel as if they are getting 24/7 care without being inpatient.
